We understand that timing, communication, and attention to detail are critical to successful breeding — and we’re committed to supporting you and your dog every step of the way.
We offer Health Clearances
We offer comprehensive health clearances, including PennHIP and Orthopedic Foundation for Animals (OFA) services. We host Full OFA Clinics twice yearly, featuring cardiac evaluations, CAER (eye exams), hip and elbow dysplasia screenings, shoulder and spine evaluations, bloodwork, microchipping, and patellar luxation examinations. In addition, we offer Partial OFA Clinics once per year, which include hip and elbow dysplasia screenings, shoulder and spine evaluations, bloodwork, and patellar luxation examinations.

Heat Cycle Notification
This form is to be filled out and presented before or at the time of your appointment.
Fresh Chilled Semen Shipment Request Form
PLEASE NOTIFY US 5 DAYS PRIOR TO YOUR REQUESTS SHIP DATE. This form is to be completed by the female’s owner BEFORE the semen will be shipped. We do not ship on Fridays.
Frozen Semen Release Form
This form is to be completed by the semen owner to give authorization to release frozen semen for breeding, transfer ownership of semen, or transfer semen to another storage facility.
Frozen Semen Shipment Request Form
Please contact us as soon as the female begins her heat cycle, so we have adequate time to prepare your shipment. At minimum, we ask for a 5-day notice prior to shipping. This form is to be completed by the female’s owner BEFORE the semen will be shipped. We do not ship on Fridays, and we do not accommodate same-day or next-day shipping.
C.L.O.N.E Buffer Order Form
Please use this form below to order our CLONE Freezing, Chilled, and Activator Buffers. Once your order is placed please allow 2-3 for processing. We do not ship out on Fridays.
Chilled Semen Kit Request Form
When a male is available and fertile, semen can be collected, chilled, and shipped for breeding. This process is made simple and economical with our CLONE Buffers. Our chilled semen kits are designed for owners, breeders, and veterinarians, and contain everything you need to collect, prepare, and ship semen safely and effectively. Once your order is placed please allow 2-3 for processing.
Contract for Storage of Frozen Semen
This form is required when you choose to freeze semen from your stud dog. A completed contract is required for all new dogs prior to your appointment.
Semen Discontinuation Form
Use this form to discontinue semen storage service.
